Study Abroad COVID-19 Acknowledgement Form


Due to the ongoing concerns around COVID-19, there are enhanced risks associated with travel and your study abroad program. The College of St. Benedict and St. John’s University (“CSB+SJU”) continually monitor the U.S. State Department, the U.S. Centers for Disease Control and Prevention, the World Health Organization and local governmental health ministries to help guide our study abroad programmatic decisions. However, while CSB+SJU strive to make your study abroad programs as safe as possible, all risks cannot be eliminated or anticipated on your program.
So that you can make an informed decision about your participation in your program, we ask you to acknowledge your understanding of the enhanced COVID-19 risks associated with study abroad by reading the information provided below and by signing this COVID-19 Acknowledgement Form.

As a CSB/SJU student, who is voluntarily participating in a study abroad program, I understand and acknowledge that: 

  • I understand I must be fully vaccinated against COVID-19, including boosters if I am eligible, in order to travel on a school-sponsored international program in 2022-2023.
  • I have read the CSB+SJU semester or short-term study abroad programs cancellation policies (if applicable).
  •  I understand that COVID-19 outbreaks is a fluid situation with changing government travel regulations. Government travel restrictions can change with little or no notice. I understand I am responsible for monitoring and adhering to government regulations for my program location(s) by reviewing the U.S. State Department Travel Advisory & the U.S. Centers for Disease Control and Prevention .
  • I understand that it is possible to contract COVID-19 and other infectious diseases even if I follow all of the safety precautions and guidelines recommended by the CDC and other governmental health officials and I acknowledge that CSB+SJU cannot and does not guarantee that my participation in the program is safe or that circumstances will not change.
  • The program location(s) may require you to take a COVID-19 test before you depart the U.S. or immediately after your arrival into the program country. If you test positive for COVID-19 due to a required test or due to symptoms, you may be delayed in traveling to your program country, or in the event you test positive upon arrival in the program country, you may have to self-isolate in your program or designated government accommodations.  Please note that in this event, CSB+SJU will be unable to refund your program fee as this money will already have been paid to our international program providers.  The CSB+SJU International Medical and Accident Insurance covers medical emergencies, but it does not cover your program fee if you are delayed (or unable) to participate in the program due to a positive COVID-19 test. You may wish, on your own, to investigate additional trip Cancelation/Interruption insurance that might cover these events.
  • I understand that if I am isolated due to COVID-19 or any other disease, my instructor will do their best to provide alternative accommodations to enable me to complete the course, but any experiential elements of the program will not be duplicated. I will be ineligible for refunds of any unrecoverable costs.
  • I understand my program or local government entity may have specific safety policies/protocols that I am required to follow prior to arrival and while present on-site such as a required additional booster shot, pre-arrival testing, self-isolation, sharing travel and contact information upon request, face mask mandates, and quarantines. Any additional program or local government policies/protocols will be at my own expense. The terms, costs and enforcement of such policies/protocols is outside the purview of CSB+SJU.
  • I understand that there may be modifications to the program with little or no notice in response to surges in COVID-19 infection rates or in response to local governmental instructions such as possible quarantines, curfews, and group gathering limits imposed by local governmental authorities and/or on-site partners.  This may result in modifications to the program calendar, weekly schedule, classroom environment, course delivery, housing arrangements, internship placements, transportation policies, and inter-continental or in-country travel restrictions. I must abide by any of these modifications.
  • I understand that cancellations and disruptions to travel arrangements, as well as local and national restrictions may limit or adversely impact my ability to arrive to – or depart from – my program, as well as possibly inhibit my movement within my program’s country.
  • CSB+SJU will not be financially liable for unrecoverable program fees, return travel costs, or other personal expenditures lost as a result of any program modification.
  • I understand that the international insurance coverage provided by CSB+SJU does not cover evacuation or other accommodations due to disruption to travel plans resulting from a pandemic. Coverage does include the ability to seek recovery of medical expenses incurred resulting from the treatment of COVID-19 but does not extend to full losses for travel and accommodation for self-isolation/quarantine expenses. I understand and acknowledge that I am solely responsible for any expenses not covered by insurance.
  • I understand that arrangements for continuity of care and/or treatment of any existing medical or mental health conditions, whether covered by personal insurance or otherwise, are my sole responsibility and should be made prior to the start of my program, and should account for possible restrictions put in place.
  • I understand that United States Consular Services and American Citizen Services may be unavailable, restricted, or cancelled without notice, in my program’s destination.
  • I understand that CSB+SJU does not have the resources to support me should I leave the country for independent travel outside the official program dates. Unintended delays or isolation/quarantine due to independent travel will not be considered an excused absence from academic programs. These unexcused absences may affect my final grade.
I have read the foregoing representations and cautions and understand that along with other risks there are specific COVID-19 risks associated with participating in am international sponsored program at this time. As witness by my electronic signature below, I agree to assume those risks and affirm my voluntary decision to participate in the Study Abroad Program of CSB+SJU made effective with the date of my signature.
