Schedule at least 6 weeks before departure.

CSB and SJU Health Services provide pre-travel consultation for CSB and SJU students. Please be aware that Health Services does not carry all the vaccines that are recommended or required for travel (see list below).

1. Pre-travel health forms must be completed before your appointment.

• Log into: Patient Portal
• Complete these three forms:
  1. Travel Consult Form
  2. Immunization Screening Questionnaire
  3. TB Screening Intake

2. After Health Services receives your three completed pre-travel forms, they will contact you to schedule a 20 or 40 minute appointment.

3. Items to bring to your appointment:

• Immunization Record
• Insurance Card
• List of current medications

Available Vaccines:

• Hepatitis A
• Hepatitis B
• Influenza
• Meningitis
• MMR-Measles, Mumps, Rubella
• Tdap-Tetanus/Diphtheria/Pertussis
• Typhoid

Note: Anti-malaria medications can be prescribed.

Vaccines not available at CSB and SJU Health Services:

• Japanese Encephalitis
• Polio
• Yellow Fever
