Before coming to campus, you are required to complete the CSB and SJU Health Form, which includes your current immunization records.

Due Dates:

• June 15 (if starting classes in August)
• February 1 (if starting classes in January)

If you miss the due date, a hold will be placed on your account which will enable you from registering for classes.
