Dietary Accommodation Forms
The College of Saint Benedict and Saint John’s University provides reasonable accommodations to students with documented disabilities and medical conditions. While all student residents are required to purchase a meal plan, those diagnosed with severe food-related conditions may require accommodations to our meal plans.
Federal disability legislation requires CSB/SJU to provide reasonable accommodations for individuals with documented disabilities. The Americans with Disabilities Act defines an individual with a disability as “a person who has a physical or mental impairment that substantially limits one or more major life activities.”
In an effort to determine appropriate accommodation eligibility, Student Accessibility Services (SAS) must obtain comprehensive information regarding a student's dietary needs.
Procedure for requesting dining accommodations:
- Complete our Request for Dining Accommodation Form.
- Have your health care provider complete our Practitioner Verification for Dining Accommodations Form.
- Initial Appointment: Call 320-363-5245 to set up an initial appointment with a Student Accessibility Specialist.
SAS will utilize information provided in the three steps mentioned above to determine accommodation eligibility. SAS frequently needs to consult with professional staff members from our Dining and Culinary Services. In such circumstances, all documentation will be stored in the SAS office and identifying information will remain private.
