Student Travel Survey

CSB/SJU asks that you submit this form any time you intend to travel.  Please complete the form pre-travel when possible, however the form can also be completed post travel .   

The data collected will allow CSB/SJU to assess our community’s travel patterns should the information be deemed helpful during the pandemic.   

This form is optional, and none of the questions in this survey are required.  You may submit as much or as little of the information requested in this form as you are comfortable sharing.  

This form should not be used to CSB/SJU club travel within the continental United States or institutionally sponsored international travel.  Please contact the Center for Global Education ([email protected]) to report institutionally sponsored international travel.

Where did you travel?

(car, airplane, train, cuiseship)

Any time you travel you should:  

  • Wear a mask.
  • Practice social distancing.  Social distancing means staying out of crowded places, avoiding group gatherings and maintaining a distance of 6 feet or 2 meters from others when possible.
  • Wash your hands often with soap and water for at least 20 seconds.  If soap and water are not readily available, use an alcohol based hand sanitizer that contains at least 60% alcohol.
  • Be diligent about self-monitoring.  CSB/SJU has a created a Daily Symptom Screen to assist you with this.

If you have spent time with someone who has tested positive for COVID19 or if you have visited a location identified by the CDC as a Level 3 Country: 

  • Stay home in quarantine for 14 days from the time you return from travel
  • Monitor your health on a regular basis

If you get sick with a fever (100.4°F/38°C or higher), cough or have trouble breathing: 

  • Seek medical advice.  Call ahead before you go to a doctor's office or emergency room.
  • Tell your health care provided about your recent travel and you symptoms.
  • Avoid contact with others.  If you must go out, wear a mask.
  • Do not travel while sick.
