This form is used to request a license for a Zoom Webinar. Please complete this form if you'd like to utilize Zoom's webinar feature for a presentation.

A form submission does not guarantee a webinar license will be assigned. You will be contacted if there are no webinar licenses available for your selected date.

2. CSB and SJU DIGITAL COMMONS

Before a video can be placed on Digital Commons, a release form (link listed below) must be filled out and signed by each non-CSB and SJU panelist. The webinar organizer is responsible for obtaining release forms and submitting them to Instructional Technology.
