1. Course renumbering (if no level change), or a change in title or a change in prerequisites can be handled by Dave Lyndgaard and the Associate Dean for Budget & Planning and does not require committee action.

2. If a course has been revised and is more than 50% new, this form should be completed.

1. Complete this form by filling in the appropriate information then click submit at the bottom of the form.
2. Submit this form by the following deadlines:

If course is scheduled to take effect in Spring or Summer the deadline is September 1 of the year before (For Example: Spring 2015 revisions must be submitted by 9/1/2014)

If course is scheduled to take effect in Fall the deadline is December 1 of the year before revision is to take effect (For Example: Fall 2014 revisions must be submitted by 12/1/2013)

THE ACADEMIC CURRICULUM COMMITTEE CANNOT GUARANTEE TIMELY REVIEW OF PROPOSALS RECEIVED AFTER THE DEADLINES.  FURTHER, IT IS THE POLICY OF THE CURRICULUM COMMITTEE TO GIVE PRIORITY TO PROPOSALS THAT ARE THE RESULT OF PROGRAM REVIEW.
