Forms Portal - Login
The Forms Portal is accessible after February 1 to students enrolling for Fall Semester and after September 20 for students enrolling for Spring Semester.
Students enrolling prior to those dates will receive an email on or shortly after those dates with instructions for setting up a CSB+SJU Network Account and accessing the portal. Students enrolling after those dates will receive the email within one week of enrolling. If you have not received the email instructions within seven days, contact the Admission Office.
Use the information in the email to set up your CSB+SJU Network Account before accessing the Forms Portal. Get instructions for setting up your Network and Email Account Password.
