Application Procedure Checklist

1. Complete the application forms and mail them to:

• YTM Code of Conduct & Parent/Guardian Permission Form
• YTM Health Release Form
2. Registration down payment:  $175.  Non-refundable upon acceptance into YTM. Remaining $175 payment due upon arrival.
3. Include a 250-500 word essay outlining your church or community leadership experiences and why you want to attend YTM.
4. Registration deadline is April 18, 2008.
5. My parent(s)/guardian will attend the June 21st Luncheon and Eucharist
