Criminal Background Check/Diocesan Sexual Misconduct Form

Because the summer YTM program involves adolescent participants, proof of a criminal background check must be on file with us before attendance at the YTM Summer Institute. If your diocese has a sexual misconduct policy, then we must also receive a signed copy of this form for our records.
