W-9

If you are making a purchase from a vendor or a service provider that our institutions have not previously paid, you will need to ask them to provide a W-9. 

A Form W-9  is a document issued by the United States Internal Revenue Service (IRS) for taxation purposes.  W-9's are collected by institutions -- such as CSB/SJU --that pay vendors or hire independent contractors (speakers, singers, etc).  The form is used to gather information about the vendor or contractor and must be on file in the CSB/SJU Business Offices before a payment can be made.
