Academic transcripts are issued to current students without charge. Transcripts are issued to former students for a fee of $3.00 per transcript through December 31, 2009. Beginning January 1, 2010, the cost will be $5.00 per transcript. Please make checks payable to the Registrar's Office, CSB/SJU. No transcript will be released to any student who has not settled all financial obligations to CSB/SJU. A hold may be placed on transcripts until all tuition, fees, room and board are paid in full.

Contact the Office of the Registrar to request a transcript. The Family Educational Rights & Privacy Act of 1974 requires this request be in writing or in person.

There are two ways to request transcripts:

1. In person at CSB/SJU.

You may fill out a transcript request form at the Office of the Registrar along with the appropriate fees, if applicable. Checks should be made out to Registrar's Office, CSB/SJU. You may also fill out and print the transcript request form provided online in advance of your visit to CSB/SJU.

NOTE: Transcripts may be requested only by the individual to whom the transcript belongs. You may not request another person's transcript.

If the transcript will be picked up in person, we require a photo ID (MN driver's license, CSB/SJU ID card or other photo ID). We will only release the transcript to the student or to someone authorized by the student in writing.

2. In writing.

An individual may request a transcript in writing. Please provide the following information in a written request (or fill out and print the transcript request form):

Full name (include all previous names)
Social Security Number
Date of birth
Your permanent address
Phone number
Dates of attendance
Original signature of individual
Address where transcript is to be sent
Payment for transcript service

Transcripts are mailed within 3 business days of receipt of the request. The signed request may also be faxed (320-363-2714) to the Registrar's Office if time is of the essence.

We cannot accept email, or telephone requests. Please note that requests not accompanied by a signature cannot be processed.
