PLEASE NOTE: Some of these forms are printable only while others can be completed on-line for direct submission of information!  Printable forms are available in Adobe PDF format.  You can view and print it using Adobe Acrobat Reader version 4.0 or higher.  If you don't have this program, we recommend that you acquire it.  Many web forms utilize it, it is easy and fast to download, and does not take up much disk space.  To obtain a free copy of Adobe Acrobat Reader go to the Adobe web site and download a copy.

Special Registration Forms

• Data Collection form for Continuing Education Students - for persons wishing to take 11 credits or less class at CSB/SJU while working fulltime.  See Course Catalog for guidelines
• Data Collection form for New or Readmitting Graduate Students - for Graduate students taking a class at SOT
• Data Collection form for Prep Students - for Prep students wishing to take a class at CSB/SJU
• Data Collection form for Special Students - for part-time adult learners not qualifying under the Continuing Education guidelines.  See Course Catalog for further information. 
• Data Collection form for Special Diocese program - for students taking special courses through the Diocese.
• Data Collection form for Employees & Spouses - for employees/spouses taking a class at CSB/SJU
  (must also submit Appropriate Class Authorization Form below).  Please turn in both forms together to the HR office.

• CSB/OSB Employee Class Authorization Form - for employees taking a class at CSB/SJU

• CSB/OSB Spouse Class Authorization Form - for an employee's spouse taking a class at CSB/SJU
  (must also submit Data Collection Form for Employees & Spouses above).  Please turn in both forms together to the HR office.
