• Click on Enter Secure Area for either SJU or CSB
• Enter your Banner ID number and your WSC code (if you do not know your WSC code, please contact IT Services for assistance)
• Click Login
• Click on Student Services
• Click on Student Records & Account Information
• Click on Enrollment Verification Information
• Select a Term from the dropdown menu
• Enter your Insurance Policy Information if you are using this for insurance purposes
• Print the form
