Degree Application Instructions

• Fill out required information

• Staple a copy of your ontrack.  This serves as a record of your current status of core requirements.

• Print a form for your 1^st major at http://www.csbsju.edu/registrar/forms/majors.htm  and complete.  (Minor forms can be printed at http://www.csbsju.edu/registrar/forms/minors.htm)
• Staple 1^st major form over Degree Application-Major and Minor page.
• Additional majors and minors should be printed out, completed and turned in separately to the corresponding Department Chairperson.

All College Honors and/or Honors Thesis (if applicable):

• Print form at http://www.csbsju.edu/registrar/impages/Appl_for_Honors.pdf and  complete.
• Honors Thesis must be signed by your Thesis Advisor.
• If completing All College Honors, please turn in to Honors Chairperson (Richard White) for approval.

You must include the following with your application to each Chairperson:

• Your current CSB/SJU transcript
• Transcripts for any transfer work you have (you can obtain a copy of this at the SJU Registrar’s office if you have sent us an official copy.

The Departments will forward your approved application to the Registrar’s Office.  If your application is completed in the spring, an audit of your degree status will be sent to your campus PO Box when you return in August (students studying abroad will be contacted via email).  If your application is completed another time during the year, and audit will be sent to your campus PO Box within 4 weeks of receipt in the Registrar's Office.

This application will be used for the certification of your degree. It is your responsibility to file a revision if you decide not to follow your original document.  Neglect of this step may jeopardize your graduation plans.
