CSB/OSB Employee Class Authorization Form- for employees taking a class at CSB/SJU. Must also submit a Data (registration) form.
*Please turn in both forms together to the HR office.

CSB/OSB Tuition Remission - for sons or daughters of CSB/OSB employees attending CSB/SJU
OSB Traveling Tuition Remission - for sons or daughters of OSB (Abbey, Liturgical Press and University) Administrative/Faculty employees hired before July 1, 1998 attending an institution other than CSB/OSB
