College Mentor Contract
The following are rules with which every volunteer College Mentor participating in the Fast Forward Youth Program (FFYP) and any related events (field trips, campus tours, service activities, etc.) must comply at all times while working with the FFYP. All FFYP staff enforces these rules. They exist to protect everyone involved with the program and uphold the integrity of students, volunteers, staff, and the program in general.
Every College Mentor must read, understand, agree to, and sign this contract before participating in the Fast Forward Youth Program or any related events. By signing, you are obligated to comply with the rules set forth. Failure to follow these rules may lead to the termination of your privilege to volunteer with the program.
I, ________________________________, (print first & last name) agree to follow the rules and guidelines listed below while serving as a College Mentor with the College of St. Benedict & St. John’s University’s Fast Forward Youth Program.
1. I will be respectful of myself, others, and my environment while participating in the Fast Forward Youth Program.
2. I will report to all FFYP sessions and activities on time and prepared to participate.
3. I will attend every session to which I am assigned. If I am not able to attend a session, I will find a replacement among the other College Mentors. I understand that if I miss more than two (2) sessions during a semester, the Fast Forward Youth Program reserves the right to terminate my volunteer position. I will discuss any extenuating circumstances with the Director as soon as possible.
4. I will maintain the confidentiality of the students, volunteers, and staff members with whom I work in the Fast Forward Youth Program. This means that I will discuss information about a student, volunteer, or staff member only in an environment that is not harmful to him or her.
5. I will immediately report any negative or suspect information regarding a student’s emotional, social, and physical well-being to an FFYP staff member. This includes information about sexual, physical, emotional, or psychological abuse of or by the student.
6. I will not encourage or promote activities that are illegal, unethical, or potentially harmful.
7. I will not have sexual relations, romantic involvement, or physical contact (as outlined during College Mentor training) with students in the FFYP.
8. I will not have sexual relations, romantic involvement, or physical contact with volunteers or staff in the Fast Forward Youth Program while participating in FFYP sessions or activities. This includes public displays of affection.
9. I will receive approval from the FFYP Director before interacting with student participants in any way outside of scheduled College Mentor sessions or FFYP program activities. This includes any type of electronic communication (email, MySpace, Facebook, instant messaging, etc.).
10. I understand that I will be transported to and from all program activities in transportation provided by the FFYP. Personal transportation is not encouraged and is only allowed in special circumstances.
11. I agree to sign up for and attend Mid-Semester and Semester-End College Mentor Reflection Sessions when I receive notification from the FFYP staff. I understand that this is an important part of my commitment to the program.
12. I understand that I must escort or supervise my assigned student participants at all times while working with the FFYP. If circumstances do not allow me to do this, I will ask another volunteer or staff member to take my place.
13. I understand that I must be supervised by FFYP staff person at all times while working with student participants.
I agree to uphold ALL policies listed above and abide by the College of St. Benedict & St. John’s University’s Joint Human Rights Policy posted at http://www.csbsju.edu/humanrights/policy1.htm. Failure to adhere to any of the above will result in the termination of my tenure as a College Mentor in the Fast Forward Youth Program. If necessary, further action will be taken.
LIABILITY CLAUSE
I understand that the Fast Forward Youth Program, sponsored by the College of St. Benedict and St. John’s University (CSB/SJU), is a “hands-on” service organization. Therefore, I agree to release CSB/SJU and the Fast Forward Youth Program from any and all liability, claims, and demands of whatever kind or nature either in law or equity, which may arise or may hereafter arise from my volunteer experiences. In addition, I hereby expressly and specifically assume the risk of injury, illness, death, or property damage resulting from transportation and activities as a volunteer with the Fast Forward Youth Program.
