Health Form Frequently Asked Questions

Q:  When is my health form due in your office?

A:  The health form is due in our office on or before August 1, 2009.

Q:  I've lost my health form. Where can I get another copy?

A: You can download a PDF version of the health form.

Q:  What immunizations are required?

Minnesota Law (M.S. 135A.14) requires that specific students in post-secondary schools provide proof of vaccination against diphtheria, tetanus, measles, mumps, and rubella allowing for certain specified exemptions (medical exemption signed by a physician or conscientious exemption signed by a student and notarized). International Students: a tuberculosis mantoux (TB skin test) must be administered within four weeks prior to attending college.

Q:  Is a physical exam a requirement upon entering CSB?

A:  Yes. The College of Saint Benedict requires all students to have a current physical exam upon entering the college. This exam must be completed by a medical doctor, a nurse practitioner, or a physician's assistant. If you are a transfer student and have a physical exam on file at your previous school, you do not need to have another exam. Please obtain a copy of this exam and forward it to our office. If you are a transfer student but did not have a physical exam prior to admission to your previous school, a physical exam is required by CSB.

Q:  If I've had a physical within the past year, do I need another one?

A:  No. As long as it has been less than 12 months since your last physical exam, all you need to do is provide us with the requested physical examination information. ATHLETES PLEASE NOTE: If you have completed a physical within the past 6 months, but plan on participating in intercollegiate athletics, your health care provider must approve you for intercollegiate participation on the CSB health form by checking the appropriate box on the first page.

Q:  Is health insurance a requirement of CSB students?

A:  Yes. All students must show proof of insurance including the insurance company, the subscriber's name (policyholder), and group/policy number. If a student does not have health insurance, she may purchase student health insurance made available by the College of Saint Benedict.
