Information for New Students

  1. Dear CSB Student,

On behalf of the CSB Health Services staff, welcome to the College of Saint Benedict! CSB Health Services is designed to meet the specific needs of the CSB women and can provide the majority of your health care needs while attending CSB. The mission of CSB Health Services is to provide friendly, quality, affordable health care at a convenient on campus location.

As part of your process prior to coming to the campus you are required to have a physical exam and review of your immunizations. These materials are due on or before June 15 for Fall semester admission or February 1 for Spring semester admission.

Please follow the steps below:

  1. Download the health form.
  2. Print out the Health Form.
  3. Complete page one and take the entire form (all three pages) with you to your appointment for your physical and immunization review.
  4. Read important information about Hepatitis and Meningitis vaccine by ctrl clicking over the word.

A special note to NCAA sports participants. Make a copy of your completed forms, prior to mailing the original to Health Services.

CSB Health Services is closed from early June to early August. If you have questions, please refer to the CSB Health Services website.

A word about the deadline for this form - the consequence of not meeting the deadline is a registration hold for the student's subsequent semester. In other words, turning this form in late will NOT impact the student's enrollment status for the first term of enrollment. If you are unable to complete this process due to special circumstances, please call CSB Health Services for assistance. Although the CSB Health Center is closed early June to early August, staff members will be processing these forms and returning calls throughout the summer.
