Due dates for nominating a student or a team:
Individual nomination forms due Friday, February 10, 2006 by 4:30 P.M.
Team nomination forms are due Friday, February 24, 2006 by 4:30 P.M.
Forms must be completed by both the nominating supervisor and nominated student/student team member in order for the nomination to be complete.
Supervisor Forms:
The team nomination needs to be turned in as one complete document (supervisor form and all student statements). No on-line nomination forms are available for student team members.
