All SJU students (undergraduate and graduate school) who are registered and physically attending classes on CSB/SJU campus are eligible to enroll in the student health insurance through Student Assurance Services (SAS). The SAS accident and sickness insurance plan is underwritten by Columbia Life Insurance Company.
How do I enroll for insurance coverage?
To enroll in the SAS Insurance Plan you can download an enrollment form and a brochure from the following website, www.sas-mn.com, or stop by the St. John's Health Center for this form and information. The insurance premium can be charged to your student account, or you can present a check for payment of the full amount. There is an open enrollment period for SJU students between 8/10/2007 and 9/18/2007 for fall semester, or between 1/15/2008 and 3/1/2008 for spring semester. If you enroll after 8/10/2007, the policy will go into effect as of 12:01 AM of the next day, after you have turned in your enrollment form. Am I automatically enrolled in the student health insurance plan if I am an international student?
Yes. All international students attending St. John's University will be automatically enrolled in the SAS plan every year and the premium charged to their student account. International students must show proof every year if they carry comparable insurance and do not require the SAS plan. You must present proof of insurance before the 9/18/2007 deadline for fall term in order for your account to be credited. This must be done every year. An international student with the SAS plan who is participating in intercollegiate sports must also sign up for the Optional Sports Coverage and must request this through St. John's Health Center. The additional premium of $900 will be charged to the student's account.
If I had the SAS insurance last year am I automatically enrolled this year?
No. You must renew (fill out an enrollment form) each year. If you are participating in intercollegiate sports you must also renew the sports coverage.
What does this insurance policy cover?

How much does the student health insurance cost?
The rates for the 2007-2008 school year for student only are: 8/10/07 - 8/09/08 $880 per full year, 8/10/07 - 12/31/07 $333 per fall semester, 1/1/08- 8/9/08 $572 per spring/summer, 6/1/08 - 8/9/08 $214 per summer. Students must be enrolled in the Basic Injury and Sickness Insurance Plan in order to purchase Optional Intercollegiate Sports Coverage. This additional coverage, for $900, meets the NCAA $75,000 requirement to cover athletically-related injuries. Sports coverage terminates when your basic accident and sickness plan terminates. Call the Health Center at 320-363-3142 or SAS at 1-800-328-2739 with questions about rates for 'student and dependent(s)' coverage. SAS insurance is not sold month by month.
Will I receive an insurance card from SAS?
Your enrollment form and premium will be processed as soon as possible. You will be issued a personalized ID card within approximately 10 business days after processing. The ID card will be mailed to your P.O. Box at SJU unless otherwise specified.
When do I need to complete a claim form?
A claim form should be filled out each time you are seen for a "new" reason. If you are being seen for a "follow up" appointment you will not need to submit a form. If you receive any medication(s) from the pharmacy, give the receipts to the Health Center staff who will attach them to your claim form, and mail them to the insurance company. SAS will process your claim and will send you a check to reimburse your medication costs. This check can be turned in to Student Accounts to offset the charge on your account.
