YOU MUST PRINT OUT THIS PAGE AND SUBMIT IT TO THE OFFICE FOR EDUCATION ABROAD (ACADEMIC SERVICE BUILDING 104, CSB) BY JANUARY 22nd, 2013  at NOON.

AGREEMENT AND WAIVER SIGNATURE FORM

 

By signing this document you authorize the program faculty director, assistant program director, and the staff of OEA at CSB/SJU to access your academic and disciplinary records and are confirming that you have read, understand, and agree to the terms on the following:

  1. 2013-14 Study Abroad Application (submitted online)
  2. Study Abroad Agreement and Waiver
  3. Study Abroad Student Handbook
  4. Study Abroad Cancellation Policies
All of the above documents are available for download from this website and paper copies will be made available to any applicant upon request.  

    I have read and understand the above documents and I agree to all terms and conditions of these documents made effective with the date of my signature recorded below.

    *Parent or Guardian signature is necessary unless the student is not considered a dependent for federal income tax or financial aid purposes.     

    Please note:

    This signed Agreement Signature Form and a set of 2 official passport-sized photos must be submitted to the Office for Education Abroad, Academic Service Building 104 (CSB), by NOON Tuesday, January 22. 

    If any part of your Application, Agreement Signature Form, Application fee, or Passport photos are not received by the Office for Education Abroad by the deadline, your application will be considered incomplete. Applications completed and submitted after NOON, Tuesday, January 22, will be considered late and will be given consideration on a space available basis only.
