Information for New Students
Dear CSB Student,
On behalf of the CSB Health Services staff, welcome to the College of Saint Benedict! CSB Health Services is designed to meet the specific needs of the CSB women and can provide the majority of your health care needs while attending CSB. The mission of the CSB Health Services is to provide friendly, quality, affordable health care at a convenient on campus location.
As part of your process prior to coming to the campus you are required to have a physical exam and review of your immunizations. These materials are due on or before June 15 for Fall semester admission or February 1 for Spring semester admission. Below is a link to download the necessary forms you will need to have completed to meet the requirements. Please download the form and follow the steps below:

- Complete page one and take the entire form (all three pages) with you to your appointment for your physical and immunization review.
- Read information about Hepatitis A, B, C as well as meningitis vaccine via the links on this page.
- Have a discussion with your health care provider about the meningitis vaccine as it is strongly recommended that you receive it prior to coming to campus. Due to the expense of the vaccine per dose, the CSB Health Services does not stock the meningitis vaccine.

A special note to NCAA sports participants-Please review and complete the shaded box "NCAA SPORTS PARTICIPATION located on the bottom of page 1.
CSB Health Services is closed from June 4th to August 8th. If you have questions, please review the FAQ document. If you need further assistance, contact our office at (320) 363-5605, Option #2, knowing that voice messages will be received and responded to sporadically during the dates that CSB Health Services is closed.

Your registration for the following semester will be held until the above information is received and considered complete.
