Accident/Injury Report Forms

Faculty/Staff/Student Workers: Use the CSB/SJU Employee Accident/Injury forms available on-line. Your supervisor must sign this form, and it must be submitted to the CSB/SJU Human Resources Department within 24 hours of the claim.

Injured Students: Faculty, staff and student workers should ensure that students injured in the course of laboratory work complete the Student Accident Report Form. The original is submitted to the Environmental Health and Safety Office and a copy to the Biology Department Safety Committee chair (Dr. Ellen Jensen).
